Schedule a medical appointment
(CA and WA residents only)
Medical appointments include diagnosis and treatment (labs, imaging, medications, etc.,) and have insurance codes on the receipt that allow for partial reimbursement of the visit (superbill).
There is an extensive intake form and patients are encouraged to send imaging and pertinent medical records in advance.
For image (MRI, X-ray, CT, etc.) review, please upload images to mymedicalimages.com. Once your consultation is scheduled, contact me to send you an invitation to this site so that you don't have to pay the membership fee. PLEASE do not send discs in the mail (I have seen too many get lost or damaged in the mail over the years.
Chose this option is you live in (or will be physically located in) California or Washington and you would like to establish a patient-practitioner relationship.
Appointments can be done in-person or online, with the exception of Fascial Counterstrain (manual therapy) appointments which are only done in-person in Washington.

Health consultations DO NOT include diagnosis and treatment and are not covered or reimbursable by insurance.
There is no intake form or other registration necessary, although many clients do chose to send a brief overview of their concerns for me to review at the time of the visit. I do not review documentation before or after the visit.
