Health consultations DO NOT include diagnosis and treatment and are not covered or reimbursable by insurance.

There is no intake form or other registration necessary, although many clients do chose to send a brief overview of their concerns for me to review at the time of the visit. I do not review documentation before or after the visit.  

For image (MRI, X-ray, CT, etc.) review/ second opinion, please upload images to mymedicalimages.com. Once your consultation is scheduled, contact me to send you an invitation to this site so that you don't have to pay the membership fee. PLEASE do not send discs in the mail (I have seen too many get lost or damaged in the mail over the years).

Choose this option if you want to have a non-medical visit or if you live outside of (or will not be physically located in) California or Washington. There is no patient-practitioner relationship established in this type of consultation as it is for educational purposes only.

Appointments are done via Zoom. I offer to record the sessions for your records as I do not offer written consults or reports for this type of visit.
